Abstract
Background: Pneumonia is a lower respiratory tract infection that causes inflammation of the lung parenchyma and excessive secretion production, often resulting in ineffective airway clearance. This condition may lead to dyspnea, ineffective cough, abnormal breath sounds, and impaired oxygenation, requiring prompt and comprehensive nursing management.
Objective: To describe the implementation of emergency nursing care for patients with pneumonia experiencing ineffective airway clearance and to evaluate changes in respiratory status following nursing interventions in the Intensive Cardiovascular Care Unit (ICVCU).
Methods: A descriptive case study using the nursing process approach was conducted on two patients with pneumonia treated in the ICVCU of Kediri Regency Hospital between December 15 and 18, 2025. Data were collected through interviews, physical examinations, clinical observations, and medical record documentation. Data analysis was performed descriptively through the stages of assessment, nursing diagnosis, intervention, implementation, and evaluation.
Discussion: Both patients presented with dyspnea, ineffective cough, retained secretions, increased respiratory rate, and rhonchi, leading to the nursing diagnosis of ineffective airway clearance. Nursing interventions included airway management, respiratory monitoring, effective coughing exercises, oxygen therapy, semi-Fowler positioning, and collaboration in administering bronchodilators, expectorants, and mucolytics. Following 2–3 days of nursing care, both patients showed reduced dyspnea, improved sputum expectoration, decreased respiratory rate, increased oxygen saturation, and improved respiratory status, although the nursing problem was only partially resolved.
Conclusion: Comprehensive emergency nursing interventions effectively improved airway clearance and respiratory status in patients with pneumonia, supporting better clinical outcomes despite partial resolution of the nursing diagnosis during the observation period.
